Alpha has the right idea. I was working up some of these ideas for a L60ish doc twink.
Chalice is pretty boring and only gives -1.
Exarch is boring, people assume you're a gimp, and only gives -1.
The rest are a must, and tbh, I would keep them on until you hit the time when you can go better -cost.
That means:
L100 (most of this works for a L60 too, but yet is still right for a L100-149)
Predator Armor Gloves
Temporal Chalice (MAYBE, or swap in/out with something like CDRs)
Hardcore CPU Upgrade (BEST you can get on)
Cure for Baldness (I still have the one you loaned me if you want this
Elysian Protector's Cuirass
Exarch Robe (IF you can handle it ^^)
... Your armor of choice in the remaining areas ... Tier is best for ACs ... otherwise find mods you like ... you might want to look at +nano armor like Bronto Hide, or +nanoskill armor, etc
L150:
Reactive Armor Sleeves, Boots, Pants (head, chest, gloves, back stay the same as above .. yes ... they really do)
L175:
Breastplate of the Azure Reveries (Elysian Protector goes away, Azure has -10cost)
L180:
Spirit Shroud of the Exterminator Unit (best back item in-game IMO, rare, but you can swap its mods for the other Spirit Shrouds whenever you need ... for instance the Control version will cut down crits on you when soloing, etc)
L190:
Jathos Molybdenum Sleeves, Boots, Pants, GLOVES (finally got rid of that Predator stuff)
Faithful Doctor Shoulders (not a -cost item, but best shoulders for a doc)
L210+:
Combined Paramedics EXCEPT:
Jathos chest (for +HP but less -cost)
-OR-
Azure chest (less +HP but best -cost)
Paramed boots
-OR-
Jathos boots (unless you're trox, they're more +HP and almost same -cost)
-OR-
Leo's Boots of Ancient Gold (if you can keep them from OEing, AND don't need to swap in your Inferno boots ... so you want the Inferno ring)
220+: Depending on your breed, you may cap your -cost fairly easy/early (trox: -45% cap, Opi/Soli: -50% cap, NM: -55% cap). As you start capping it, you can consider things like removing Azure for Paramedic, or putting a NCU in instead of the CPU upgrade.
Don't forget that there are other things affecting your -cost ... Org city advantages, implants/symbs, and you can get outside buffs from MPs and NTs. For instance, with a CPU upgrade, 4 pieces of Paramedic, and city advantages ... Hyde is capped at -45% and never needs to ask for OSBs. Or, if he wants, he can swap out the CPU upgrade and get -cost buffs. Harder to do that on Opi/Soli/NM (and that's a GOOD thing for them btw, not a trox advantage) but quite doable.

Now, here's the kicker ... none of the stuff above is easy to get.
Elysian Protector: semi-rare drop from the imps in Ely that drop Focus-Funneling Spirits ... NOT NODROP, buyable
Reactive: From the RK witch bosses that you kill for Biodome Keys ... NODROP
Azure: Mercs ... NODROP
Jathos Molybdenum: Biodome ... NODROP
Spirit Shroud: rare drop from Penumbra Notum Scourages ... NODROP
Combined Paramedics: Buyable, and the cheapest of the AI armors, but still quite expensive and high reqs ... QL300 armor takes QL192+ Spiritual & QL240+ Enduring lead bots
Leo's Boots: Zods (somewhere between 40 and 120 Z points depending on who is there).
Cure for Baldness: I still have the one you gave me, and don't use it anymore ... want it?

PS. For implants/symbs there are 3 important but overlooked stats to work on buffing besides nanoskills and -cost:
Psychic
Nanodelta
Healdelta
See this thread for why. If you do everything you can to lower your nanocost, raise your Psychic, raise your healdelta/nanodelta, buff your nanoskills as needed, and max your nanoinit (oh, and use /assist ... sad seeing TL6/TL7 docs that don't know about "/macro assist /assist") ... you will be the best doc you can be. ACs and Weapons come second and later in life.

I didn't say worthless. And IIRC (been awhile) Merlins has a couple of good -cost pieces too. As long as you have your Hardcore CPU Upgrade in, your merlins, and get cost from MP/NT ... you should be pretty good on your -cost.
Maxed? Maybe not ... but ya gotta have goals eh?
